Surgical Repair of Ruptured Thoracoabdominal Aortic Aneurysm in a Renal Allograft Recipient

The surgical repair of a ruptured thoracoabdominal aortic aneurysm in a 59-year-old man with a transplanted kidney is reported. To reduce detrimental sequelae to the kidney resulting from ischemia during simple crossclamping of the aorta, perfusion of the renal allograft was ensured by using an aorta-to-left-femoral-artery bypass with a centrifugal pump. The employed method was found to be a safe and efficacious means of circulatory support to the renal transplant in the patient described here. Thirty-six reports (concerning seventy patients) of aortic repair in renal transplant recipients are briefly reviewed with a discussion of management alternatives.
