Department of Surgery, University of Bologna, School of Medicine, Bologna, Italy Although internal iliac artery aneurysms are uncommon, their timely recognition and surgical treatment are important since, like all other abdominal aneurysms, they tend to enlarge and rupture, often without warning, and the outcome is usually fatal. An aneurysm of the left internal iliac artery, in a seventy-six-year-old obese man, which ruptured into the rectum, was successfully treated by partial removal, distal aneurysmorrhaphy, and positioning of an aortofemoral prosthetic bypass.
