Departments of Thoracic and Cardiovascular Surgery and Medicine, Nassau Hospital, Mineola, New York This is the first reported case of an iliac artery-iliac vein arteriovenous fistula secondary to rupture of an iso lated atherosclerotic iliac artery aneurysm. The patient presented with systemic and local signs of a large fistula. The operative treatment of fistulas between large vessels has been associated with a high mortality rate because of massive, uncontrolla ble hemorrhage. For this reason an autotransfuser was used. The opera tion was performed with only 1 to 2 minutes of hypotension, and the post operative result was excellent.
